Application for Degree

The Office of the Registrar requires that rising seniors submit their Application for Degree form two semesters prior to completion of degree requirements.  For ease in completing this process, log in to Student Planning and select View My Progress which will display your general education and major/minor course work that should be entered on the form. 


Helpful Tips and Examples:

Complete the core curriculum (formerly general education) requirement section as follows:

 

Complete the major/minor sections by listing only the courses you still need to complete.

Include outstanding incomplete and/or in progress (grades of I or IP) courses.

Include any and all courses that are approved as substitutions. 

Include courses that you may transfer from another institution to satisfy remaining degree requirements.

You’ll need to print one copy on blue paper (available in the Registrar’s Office). Be sure to save a copy for your records and forward a copy to your advisor. The blue printed copy needs to be signed the Department Chair / Program Coordinator and your advisor. This completed signed blue copy needs to be turned in to the Registrar’s Office two semesters prior to your anticipated graduation.

IMPORTANT: The Application for Degree form is an editable PDF file, which means that if you have the latest version of Adobe Reader installed on your computer, then you can type information in the PDF form and save your data. Adobe Reader is free and available for download. If you already have Adobe Reader installed on your personal computer and want to update it, click the Help menu, and select Check for Updates.

Internet Explorer Users: You should be able to open the form in Internet Explorer, type your information into the form, save the form data, and then print the page.

Google Chrome Users: If you are using Google Chrome, then you’ll need to save the PDF form to your computer first and then fill out the form that’s been saved to your computer. (If you fill out the form directly in Chrome, you won’t be able to save your data.)
