Students who plan on getting a sports physical done at the Student Health Center should complete the intake form and be aware of the following prior to making an appointment:
- If a student has had any sort of surgery/hospitalization/injury for which they had any sort of medical evaluation in the past year, the Student Health Center needs documentation that whomever they saw for that issue has cleared them to participate in sports. Also, if they have any ongoing medical condition that they see a health care provider for regularly (diabetes, asthma, etc.), it is best for that provider to complete the sports physical, as they can attest to whether that ongoing medical condition is adequately controlled.
- If a student wears glasses or contacts, they must wear them for their sports physical. The SHC will not be able to sign off on their physical if their vision is less than 20/50.
- Each student is required to give a urine sample for a sport physical to check for protein and/or glucose in the urine.
- Physicals are recommended being completed at least 1 month in advance of the first day of sports participation in case something comes up that requires further testing/evaluation.
- The purpose of the physical is not just to sign a piece of paper, it is to make sure that students are healthy enough to participate in athletics, and to identify health problems that could interfere with sports performance and/or increase the chances of injury.

FAQs
- Can I get COVID tested in the Student Health Center?
The COVID test available in the Student Health Center is an antigen (rapid) test. This test is only accurate if you are having symptoms, AND if your symptoms have started within the past 5 days. If you have no symptoms, or if your symptoms have lasted longer than 5 days, the antigen test will not give accurate results, and it is recommended that you have a PCR COVID test done. The Student Health Center can do a PCR test sample; however, the sample will then have to be sent to OSF lab which is not covered under your student health fee. You can also contact Eagle View Community Health System (can apply for a sliding scale to reduce cost) or Walgreens in town to schedule a PCR test. Either option, insurance is billed; however, you may still have to pay a fee out of pocket depending on your coverage.
- Do I have to schedule an appointment?
While appointments are encouraged and are given priority to be seen, we do accept walk-ins as well. To schedule an appointment at the Student Health Center, please call (309)536-6055, or use OSF My Chart to schedule an appointment.
The Student Health Center is located on the main floor of T.H. McMichael Residence Hall, at 316 N. 9th St.
- Who is eligible for services? Is there a cost for your services?
All full-time students registered at Monmouth College are eligible for services. While the Student Health Center offers a variety of services at no cost, certain medical expenses such as prescriptions, x-rays, and any off-campus testing are the student’s responsibility. If the student has medical insurance, we will file any charges not covered by the student health fee with the student’s insurance. For students who have no insurance, please see “RELATED LINKS” in red on this page to find options for financial assistance.
- Can someone else access my student health record?
All health services records are confidential in accordance with Illinois law. Student records can only be released to a third party with a student’s written consent (done through the Student Health Center) except under specific circumstances (threat of harm to self or others, court-ordered subpoena, or child or elder abuse). Records will not be a part of the student’s academic record.
- Can the Student Health Center prescribe medication?
The Student Health Center providers are able to prescribe medications, if needed. If a student has been prescribed medication prior to coming to Monmouth College, we encourage them to get enough refills to cover the semester.
- Who do I call if I am concerned about the safety of a student?
If you are concerned about the well-being of a student, contact Campus Safety at (309)457-3456.
- How do I seek medical attention after clinic hours?
For medical concerns that are not an emergency, but the student feels they need to be seen before the clinic opens, OSF Specialty Clinics (309)734-1543 provides primary care and urgent care services, and is open Monday-Friday, 7am-6pm, and on Saturday 8am-5pm. *Note: this option is not covered by the student fee.
In the event of a medical health emergency, the student needs to call 9-1-1 for an ambulance to take them to the emergency room. OSF Holy Family Medical Center (309)734-3141 is located in Monmouth close to campus, and is available 24 hours a day.
