All requests for transcripts, whether official or unofficial, must be made in writing (with a signature) to the Office of the Registrar. The written request (including your signature) can be submitted to our office by using any of the following methods: postal mail, fax (309-457-2235), in person or emailed as a scanned attachment to firstname.lastname@example.org. Please allow up to 3-5 business days to process requests.
We have included two copies of the Transcript Request Form. You can choose either form.

This is an editable Adobe Acrobat file, which requires the free Adobe Reader software to be installed on your computer. You can type your information into the form, and then print the page. Remember to include your signature.
- Transcript Request Form - HTML Web Page
No special software required. Will include web page information when printed. Remember to include your signature.
Information needed in order to receive an official transcript:
- Dates of attendance at the College
- Name attended under (in case of name change)
- Social security number or student ID number
- Information on where we should send the transcript
- Transcript Request Form signed and dated
Express Mail Options
If you should need your transcript sent through express service, please contact the Registrar's Office at 309-457-2326 and we will provide you with the extra shipping charges, based on your service preference and shipping location. Once you have received a total for express shipping from the Registrar's Office, follow this link to begin the special shipping request process.
Please include a current address, telephone number and/or email address where you can be contacted. If you have outstanding financial obligations to Monmouth College your transcript cannot be released until your obligation is met.
